Research & Publications
Professor Kunadian's research career began in acute myocardial infarction (heart attack), culminating in an academic MD Degree from Newcastle University (2004โ2007). His thesis at James Cook University Hospital evaluated the reorganisation of myocardial infarction service delivery โ specifically the transition from thrombolysis-based care to a primary PCI service, including the final analysis of the landmark MERLIN randomised trial.
His scholarly output includes 29 peer-reviewed journal publications (17 as first author), 18 original articles, 5 reviews, 3 case reports, and a book chapter on adjunctive PCI technology in the second edition of Essential Interventional Cardiology. He contributed to the EACTS clinical guideline on antiplatelet and anticoagulation management in cardiac surgery (European Journal of Cardiothoracic Surgery, 2008). He has reviewed over 60 manuscripts since 2007 for leading journals including Heart, Annals of Internal Medicine and European Journal of Cardiothoracic Surgery.
Professor Kunadian's research group was the first in the UK to publish individual PCI (Percutaneous Coronary Intervention) operator outcomes โ a groundbreaking paper published in the BMJ (2008). Using cumulative funnel plots and statistical process control, this methodology has since been adopted across the entire UK by the British Cardiac Interventional Society (BCIS) for reporting individual cardiologist and hospital outcomes. His model is now a national standard used to assess performance across the NHS. He was also an invited expert presenter at the Transcatheter Cardiovascular Therapeutics (TCT) Conference, Washington DC, USA (2007).
Lancet (2014): HEAT-PPCI Trial โ Unfractionated heparin versus bivalirudin in primary PCI (529 citations)
BMJ (2008): Cumulative funnel plots for early detection of inter-operator variation in PCI โ first UK publication of individual operator outcomes
Heart (2010): Impact of evidence-based strategies to reduce door-to-balloon time in STEMI using statistical process control
American Heart Journal (2007): Final analysis of the MERLIN randomised trial โ early invasive versus conservative treatment after failed fibrinolysis
Annals of Thoracic Surgery (2007): Meta-analysis of valve haemodynamics โ stentless versus stented aortic valves (106 citations)
Catheterization & Cardiovascular Interventions (2007): Meta-analysis comparing anti-embolic devices with standard PCI in acute myocardial infarction
Book Chapter: Adjunctive PCI technology โ Essential Interventional Cardiology, 2nd edition
EACTS Guideline (2008): Antiplatelet and anticoagulation management in cardiac surgery โ European Journal of Cardiothoracic Surgery
Ongoing research at Liverpool Heart & Chest Hospital focuses on quality outcome analysis in PCI through regular formal evaluations and collaborative meetings. Current research interests include service reconfiguration in acute cardiac care, risk stratification and operator performance outcomes, and application of statistical process control in clinical pathways.
